Set in 1895, the serial follows Dr. Eleanor Bramwell (Jemma Redgrave), a gifted and determined physician whose ambitions are continually blocked by the male-dominated medical establishment. After being dismissed from a London hospital, she secures funding to open her own charitable clinic, The Thrift Infirmary, in the city’s impoverished East End. She is aided by the dependable Nurse Ethel Carr (Ruth Sheen) and the admiring, innovative Dr. Joe Marsham (Kevin McMonagle). Eleanor’s work at The Thrift is a constant source of tension with her respectable physician father, Dr. Robert Bramwell (David Calder), and draws the scorn of the influential surgeon Sir Herbert Hamilton (Robert Hardy). Her personal life is complicated by a turbulent relationship with fellow doctor Finn O’Neill (Andrew Connolly).

Bramwell set itself apart from conventional costume drama by grounding its narrative in the unvarnished realities of nineteenth-century medicine. The series was built around its unflinching surgical scenes, which graphically recreated procedures performed with rudimentary anaesthetics and a developing understanding of antiseptic practices. This commitment to historical accuracy provided a visceral counterpoint to the more traditional drawing-room conflicts. The machinery of the medical procedural served as a vehicle to examine the rigid social structures of the era, dramatising issues of class, poverty, and institutional sexism through Eleanor’s professional struggles. Created by a team including lead writer Lucy Gannon, the production eschewed sentimentality for a grittier depiction of Victorian life.
